Our easy, one-page loan application is available to you in several ways.
Loan Online: Download (PDF) and print the form and after completing it fax or mail it to us (you can use PHS interoffice mail).

Loan By Fax: We are always happy to fax you a loan application and you can fax it back to us at 419-471-1837.
*Immediate family members of Premier Advantage members receive the same loan benefits if co-signed by the premier member.
